    <abstract locale="en_US">Background: Multiple severity scoring systems have been devised and evaluated in adult sepsis, but a simplified scoring model for pediatric sepsis has not yet been developed. This study aimed to develop and validate a new scoring model to stratify the severity of pediatric sepsis, thus assisting the treatment of sepsis in children.

Methods: Data from 634 consecutive patients who presented with sepsis at Children&#x2019;s hospital of Hunan province in China in 2011-2013 were analyzed, with 476 patients placed in training group and 158 patients in validation group. Stepwise discriminant analysis was used to develop the accurate discriminate model. A simplified scoring model was generated using weightings defined by the discriminate coefficients. The discriminant ability of the model was tested by receiver operating characteristic curves (ROC).

Results: The discriminant analysis showed that prothrombin time, D-dimer, total bilirubin, serum total protein, uric acid, PaO2/FiO2 ratio, myoglobin were associated with severity of sepsis. These seven variables were assigned with values of 4, 3, 3, 4, 3, 3, 3 respectively based on the standardized discriminant coefficients. Patients with higher scores had higher risk of severe sepsis. The areas under ROC (AROC) were 0.836 for accurate discriminate model, and 0.825 for simplified scoring model in validation group.

Conclusions: The proposed disease severity scoring model for pediatric sepsis showed adequate discriminatory capacity and sufficient accuracy, which has important clinical significance in evaluating the severity of pediatric sepsis and predicting its progress.

    <abstract locale="en_US">Background: Performance is a multi-dimensional and dynamic concept. During the past 2 decades, considerable studies were performed in developing the hospital performance concept. To know literature key concepts on hospital performance, the knowledge visualization based on co-word analysis and social network analysis has been used.

Methods: Documents were identified through &#x201C;PubMed&#x201D; searching from1945 to 2014 and 2350 papers entered the study after omitting unrelated articles, the duplicates, and articles without abstract. After pre-processing and preparing articles, the key words were extracted and terms were weighted by TF-IDF weighting schema. Support as an interestingness measure, which considers the co-occurrence of the extracted keywords and "hospital performance" phrase was calculated. Keywords having high support with "hospital performance" are selected. Term-term matrix of these selected keywords is calculated and the graph is extracted.

Results: The most high frequency words after &#x201C;Hospital Performance&#x201D; were &#x201C;mortality&#x201D; and &#x201C;efficiency&#x201D;. The major knowledge structure of hospital performance literature during these years shows that the keyword &#x201C;mortality&#x201D; had the highest support with hospital performance followed by &#x201C;quality of care&#x201D;, &#x201C;quality improvement&#x201D;, &#x201C;discharge&#x201D;, &#x201C;length of stay&#x201D; and &#x201C;clinical outcome&#x201D;. The strongest relationship is seen between &#x201C;electronic medical record&#x201D; and &#x201C;readmission rate&#x201D;.

Conclusion: Some dimensions of hospital performance are more important such as &#x201C;efficiency&#x201D;, &#x201C;effectiveness&#x201D;, &#x201C;quality&#x201D; and &#x201C;safety&#x201D; and some indicators are more highlighted such as &#x201C;mortality&#x201D;, &#x201C;length of stay&#x201D;, &#x201C;readmission rate&#x201D; and &#x201C;patient satisfaction&#x201D;. In the last decade, some concepts became more significant in hospital performance literature such as &#x201C;mortality&#x201D;, &#x201C;quality of care&#x201D; and &#x201C;quality improvement&#x201D;.

    <abstract locale="en_US">Author reports a case of 67- years- old men referred to Donga University Hospital silicosis who had worked in tunnel drilling where presumably had been exposed to high concentration free silica for almost 20 yr. This report is considered the first in Korea to the best of our knowledge since no cases on silicosis could be reported in this occupation in reviewing various literature sources in Korea. The physician's first impression was lung cancer since the CT findings. However, it turned out to be pneumonia and silicosis. It was eventually diagnosed with silicosis and pneumonia, and tuberculosis. The patient is suffering from various clinical courses for long time.

    <abstract locale="en_US">Background: The aim of the study was to discuss on the influential factors of the mode of classification of partition management in the emergency triage.

Method: Retrospectively analyzing the effects of emergency triage of 156 cases who adopted the classification partition management mode during Oct 2014 to Oct 2015 in Xuzhou Central Hospital (Xuzhou, Jiangsu Province, China). They were divided into triage success group of 108 cases and triage failure group of 48 cases. Comparing the single factor analysis and multi-factor analysis, and selecting possible influential factors. 

Result: According to the single factor analysis, for the patients who came to the doctor in the daytime and working days, the higher education degree and compliance they had, the faster the back-show time of emergency inspect and check came back, the more comprehensive the body examination and disease history taking were done, the simpler the disease condition was, the higher triage success rate they received. Compared to the emergency observation time between two groups, the difference was not statistically significant. According to the multi-factor analysis, the emergency check and examination back-show time, the comprehensive degree of body examination and disease history taking and the complexity degree of disease could be the independent risk factors for triage success. 

Conclusion: Simplify the examination procedure, improve the efficiency of back-show and acquire detail disease information are important methods for the improvement of triage success.

    <abstract locale="en_US">Background: This research aimed to systematically study and outline the methods of hospital performance evaluation used in Iran.

Methods: In this systematic review, all Persian and English-language articles published in the Iranian and non-Iranian scientific journals indexed from Sep 2004 to Sep 2014 were studied. For finding the related articles, the researchers searched the Iranian electronic databases, including SID, IranMedex, IranDoc, Magiran, as well as the non-Iranian electronic databases, including Medline, Embase, Scopus, and Google Scholar. For reviewing the selected articles, a data extraction form, developed by the researchers was used.

Results: The entire review process led to the selection of 51 articles. The publication of articles on the hospital performance evaluation in Iran has increased considerably in the recent years. Besides, among these 51 articles, 38 articles (74.51%) had been published in Persian language and 13 articles (25.49%) in English language. Eight models were recognized as evaluation model for Iranian hospitals. Totally, in 15 studies, the data envelopment analysis model had been used to evaluate the hospital performance.

Conclusion: Using a combination of model to integrate indicators in the hospital evaluation process is inevitable. Therefore, the Ministry of Health and Medical Education should use a set of indicators such as the balanced scorecard in the process of hospital evaluation and accreditation and encourage the hospital managers to use them.

    <abstract locale="en_US">Leishmaniasis is an endemic parasitic disease in Iran. This paper reports the case of a 5-yr-old boy who presented with multiple isolated cervical lymphadenopathy for several months with no history of fever and no signs or symptoms. In an excisional lymph node biopsy, Leishmania parasites were histologically detected. Thus, leishmaniasis should be included in the differential diagnosis of isolated lymphadenitis in immunocompetent patients, even if the K39 and IFA for kala-azar are reported as negative.

Methods: Computer searches were conducted between Jan 2000 and Mar 2023 using the databases of CNKI, VIP, PubMed, the Cochrane Library, Wanfang and Web of Science in search of trials with randomization and control including Shaoyao decoction in UC. After quality assessment, The Revman 5.3 was applied.
Results: Overall, 23 articles were selected for the study totally. Shaoyao decoction improved clinical effective rate (P&lt;0.001) and effective rate of syndrome (P&lt;0.001), IBDQ (P&lt;0.001), interleukin-4 (P&lt;0.001) and interleukin-10 (P&lt;0.001), reduced adverse reactions (P=0.004), recurrence rate (P=0.03), DAI (P&lt;0.001), tumour necrosis component &#x3B1; (P&lt;0.001), interleukin-1&#x3B2; (P&lt;0.001), interleukin-6 (P&lt;0.001), and extremely sensitive C-reactive protein (P=0.02).
Conclusion: Shaoyao decoction has certain advantages and good safety in the treatment of UC; however the findings still need to be supported by excellent studies.</abstract>

    <abstract locale="en_US">Background: A number of important problems remain unresolved in the field of assisted reproductive technologies (ART) using germ cells from a single donor. We aimed to assess attitudes about the use of sex gamete donation in ART in different social groups including medical workers, oocyte recipients, and oocyte donors.
Methods: To achieve this goal, we surveyed 286 participants from seven countries. Of them, 190 were medical workers from ART clinics (respondents from seven countries), 45 were oocyte recipients, and 51 were oocyte donors.
Results: The main motive for oocyte donation was financial compensation, which draws attention to the social disadvantage of the donor population. Medical workers with more than 5 years of work experience (79.7%) supported the use of programs tracking donor sex cells and limiting the use of oocyte donors.
Conclusion: The willingness and consent of the surveyed medical workers and oocyte recipients to use the donor material and create a unified registry of donor sex cells demonstrates the importance of this issue.

    <abstract locale="en_US">Sugar sweetened beverages (SSBs) have become a cause of concern because of their growing consumption levels across age groups and associated chronic diseases such as diabetes mellitus, cardiovascular diseases and cancers. The aim of this review was to provide a detailed profile of the SSBs trends and associated health risk, with special focus on its role in breast cancer development. A review of current literature has depicted increased intakes of SSBs across the globe with servings ranging from 3 to 11 per day in different countries, while children, adolescents and young adults report the highest intake levels. These increased intakes further contribute to different metabolic diseases via increased body adiposity, blood glucose and insulin levels, and increased post-menopausal estrogen levels, all of which contribute to chronic diseases, including cancers. Nutrition interventions including ones that target SSBs reduction seem to have a positive impact on reducing the development of these non-communicable diseases and are also associated with better prognosis and survival chances in cancer patients. However, the implementation of SSBs taxation and mass awareness campaign interventions remains poor due to lack of policy development and regulation for these beverages. The control of SSBs intake across the world requires rigorous research to construct efficient and practical policies to reduce the accessibility and marketing of SSBs while simultaneously increasing awareness in the public regarding the health risks of these beverages. To achieve this, a coordinated approach involving different public and private sectors is needed.&#xA0;</abstract>

    <abstract locale="en_US">Background: The prevalence of sexually transmitted infections in men who have sex with men (MSM) is higher than in other communities. We aimed to Uncovering the risk of sexually transmitted infections in MSM in Indonesia.
Methods: We adopted a cross-sectional design carried out in 24 provinces in Indonesia from March-May 2023, using secondary data from the 2018-2019 Integrated Biological Behavior Survey (IBBS). The population consisted of MSM who had sex with men totaling 6,000 people. A total of 4,290 samples were selected using Respondent Driven Sampling (RDS). The data obtained were analyzed with chi-square and logistic regression models of risk factors.
Results: The prevalence of STIs in MSM was 20.9%. Marital status, consistency of condom use (P&lt;0.01), how to get condoms (P&lt;0.001), and STIs examination (P&lt;0.001) had a significant association with the incidence of STIs among MSM from 2018 &#x2013; 2019. Meanwhile, the use of lubricant during sex association had no significant relationship with the incidence of STIs. Vaginal and anal sexual intercourse had a significant relationship with the incidence of STIs, where MSM who CI95% (had their first anal sex between the ages of 25-49 years (P&lt;0.001), AOR=2.26 and vaginal sex at the age of &#x2265;50 years were more at risk of experiencing STIs (P&lt;0.017), AOR=1.33.
Conclusion: Marital status, consistency of condom use, how to get condoms, and STIs examination, vaginal and anal sexual intercourse had a significant association with the incidence of STIs among MSM.</abstract>

    <abstract locale="en_US">Background: Tuberculosis (TB) control programs confront a significant challenge in ensuring patients fully adhere to their treatment regimens. Video-observed therapy (VOT) is an alternative digital technology for monitoring tuberculosis treatment that may potentially improve adherence and clinical outcomes. However, there is limited evidence supporting the effectiveness and acceptability of VOT. This scoping review aimed to summarize the characteristics of existing evidence-based VOT for tuberculosis treatment monitoring and to describe the evidence for their effectiveness and acceptability.
Methods: This review was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) extension for scoping review protocol in Jan 2023. Three databases, namely PubMed, Web of Science, and SCOPUS, were used in this review.
Results: The search identified 170 articles, and after the identification and screening process, 22 articles were included in this review. The findings were categorized into effectiveness and acceptability.
Conclusion: There is a progressively growing body of evidence, particularly in treating and monitoring TB using VOT, which has a positive impact on improving effectiveness regarding health outcomes and is widely accepted to implement. Future studies, such as non-inferiority trials and cost-effectiveness evaluations, will help improve tuberculosis strategies and management.</abstract>

    <abstract locale="en_US">Background: Anemia is a significant public health problem worldwide in rich and poor countries. Anemia among teenagers can be seriously almost entirely caused by a lack of substance iron, which is closely related to the level of severity of anemia. The most visible effect of anemia is a decline in achievement studying in school. This article briefly introduces anemia, etiology, pathophysiology, impact, and preventive measures.
Methods: Major databases, including Scopus, Pubmed, Proquest, Google Scholar, and Science Direct, were researched to obtain articles related to anemia in adolescent girls. The keywords used in the literature search were &#x201C; anemia and teenage girls "and " anemia prevention. " The time frame of the articles obtained ranged from 2012 to 2023.
Results: Enhancement observed prevalence&#x200B; during several final years will cause prevalence to exceed&#x200B; the agreed target level. The most visible impact of anemia is a decline in achievement studying in school. Not only that, teenage women who suffer from anemia are at risk of experiencing anemia during pregnancy, which can hurt the growth and development of the fetus in Content. Apart from that, there is the potential to experience complications during pregnancy and childbirth.
Conclusion: Identifying and understanding the etiology of anemia is critical to developing effective prevention strategies with screening. An integrated approach to early prevention of anemia involves collaboration between the health, education, and community sectors.&#xA0;</abstract>

    <abstract locale="en_US">Background: Phenols and parabens have been associated with various adverse health outcomes. However, their relationship with breast cancer remains inconsistent, and the combined effect is still unknown. This study aimed to examine the association between mixed phenols and parabens and breast cancer among female adults.
Method: Participants for this study were obtained from six cycles of the National Health and Nutrition Examination Survey (NHANES). The weighted logistic regression model was employed to investigate the relationship between individual chemicals and breast cancer. Furthermore, the weighted quantile sum (WQS) regression was used to assess the joint effects of phenols and parabens on breast cancer.
Results: The study included 4993 participants, with 154 women diagnosed with breast cancer. After adjusting for all potential covariates, triclosan (TCS) showed a positive association with breast cancer (OR for Q3 = 2.12, 95% CI: 1.23-3.65), while propylparaben (PrPB) exhibited a negative association with breast cancer (OR for Q4 = 0.48, 95% CI: 0.23-0.98). The WQS regression mode found no significant difference between mixed chemicals and breast cancer (OR for positive model = 1.09, 95% CI: 0.65-1.84 and OR for negative model = 0.95, 95% CI: 0.57-1.58).
Conclusion: Exposure to phenols and parabens has distinct effects on breast cancer risk. High-quality research is essential to obtain conclusions that are more reliable and uncover potential underlying mechanisms.</abstract>

    <abstract locale="en_US">Pneumoconiosis, an occupational lung disease that arises from the inhalation of harmful dust, often coexists with multiple chronic diseases that aggravate its prognosis and complicate treatment. These chronic diseases can include pulmonary and cardiovascular conditions, as well as cerebrovascular, renal, endocrine, and rheumatic comorbidities. The underlying pathogenesis, which involves inflammatory responses, blood vessel damage, and immune system compromise, is rooted in the persistent presence of inhaled dust particles within the pulmonary alveoli and the systemic circulation. Management emphasizes preventive measures, notably through regulatory oversight to reduce exposure, and is coupled with vigilant chronic disease monitoring and lifestyle interventions. This article reviews the mechanisms, research status, and management strategies for pneumoconiosis patients complicated by the aforementioned chronic diseases. We aimed to bridge the gap between understanding the complexities of pneumoconiosis combined with chronic conditions and translating this insight into practical and effective management strategies to enhance patient care.</abstract>

    <abstract locale="en_US">Background: Idiopathic granulomatous mastitis (IGM) is a chronic breast disease with unknown pathophysiological and clinical aspects. Therefore, we designed this multi-center prospective case-control study to explore and clarify the risk factors with an acceptable sample size.
Methods: From March 2021 to December 2023, five hundred-four women with a histologically proven IGM were entered into the IGM group and 504 women with healthy breasts were included in the control group across nine centers. Participants' information was obtained by trained staff. Multivariate binary logistic regression was used to estimate the odds ratio (OR) and 95% confidence intervals (CI) for the associations between variables and IGM.
Results: The mean age of all participants was 34.92 &#xB1; 7.12 yr. The previous history of diseases as a whole, was significantly higher in the IGM group (33.1%) compared with the control group (26.6%). Logistic regression showed that previous lactation (OR= 7.51, 95% CI= 2.37-23.77) and a positive history of diabetes (OR = 4.23, 95% CI= 1.32-13.51) had a positive association with IGM, while OCP use (OR = 0.70, 95% CI= 0.52-0.93) was associated with IGM reversely.
Conclusion: The history of breastfeeding and not its duration is associated with a 7-fold increase in the rate of IGM. Furthermore, previous history of diabetes is a risk factor, while OCP use is a protective factor against IGM. We are designing another study to further investigate the relationship between breastfeeding, milk stasis, and IGM, based on our findings.</abstract>
